Children's Course Registration Information

Children's courses require pre-registration. To apply for a course, parents are requested to download following fillable pdf form.

Student application form

Use Adobe Acrobat Reader to fill the form and email the completed form to childrens-courses@mahavana.dhamma.org. Please save a copy with you so that you may reuse it in future. Printed forms can be sent to the course registrar mentioned below for each course. After we receive your completed application and permission forms, we will review your application and contact you.

NOTE: Please leave all writing materials, books, games, cassette players, etc. at home.
